Physician-assist medical record intelligence

Professional peace of mind for high‑stakes chart review.

DyZen Med transforms large, complex medical records into a citation-backed IME workbench— from Review Records through Build Draft, physician-controlled opinion recommendations, Live Draft Preview, and Narrative Export—so licensed professionals can focus on judgment, not paperwork.

Physician-assist only. DyZen Med does not replace professional judgment or make clinical decisions.

How it works

Simple workflow • complex results
1

Ingest & normalize

Upload PDFs and mixed record packets. DyZen Med extracts text (including scanned content when needed), normalizes dates, and reduces document noise so downstream summaries remain consistent.

2

Review Records

Work through the chart with structured summary and timeline views, case search, and source verification— so key facts stay traceable before you begin drafting the IME.

3

Build, preview & export

Dictate exam content, assemble findings in Build Draft, review physician-controlled opinion recommendations, watch the Live Draft Preview form, and export a Narrative Draft from approved content. AI suggestions require your review and acceptance; your judgment remains the final authority.

Security & compliance posture

Supports HIPAA‑aligned workflows

Confidentiality by design

Onboarding is governed by confidentiality terms to protect both platform methodology and participating users.

Professional control

DyZen Med is built to assist—not replace—clinical reasoning. Outputs are designed for physician review and editing, and the platform emphasizes clarity, traceability, and consistent structure over automated “decisions.”

Infrastructure & transparency

DyZen Med is hosted on AWS with TLS in transit. Subprocessors are engaged under applicable agreements for qualifying processing; supported AI endpoints are configured to request minimal retention where supported. PHI moves only through approved paths, and acceptance of the current Business Associate Agreement (BAA) is required before uploads can proceed in the platform. In the app, organizations can view compliance status and BAA history and download a compliance report (PDF). See the Security page for a concise overview.

FAQ

Direct answers • no fluff
Does DyZen Med replace physician judgment?

No. DyZen Med doesn’t think for you—it works for your time. The platform scans large medical record sets to extract factual information, organize events chronologically, and strip away duplicative or irrelevant noise that makes chart review exhausting.

By turning dense, disorganized records into clear, review-ready outputs, DyZen Med makes chart auditing faster and more manageable—so professionals can focus on interpretation, context, and decision-making where it matters most. Clinical judgment, conclusions, and final opinions always remain with the licensed professional.

What types of medical records does DyZen Med work with?

DyZen Med is built to handle large, complex medical record packets—the kind that span years, multiple providers, and countless pages. It works with PDFs, common scanned document formats, and ZIP files containing supported records.

Whether the records include clinic notes, operative reports, therapy documentation, or long-running treatment histories, DyZen Med focuses on organizing information into a clear timeline and structure, helping professionals quickly understand what happened, when it happened, and where to look next.

Who is DyZen Med designed for?

DyZen Med is designed for professionals and organizations that routinely work with large, complicated medical record sets—where clarity, accuracy, and time matter.

We also understand how overwhelming it can be for individuals to manage years of medical documentation: long cancer treatment histories, records spread across multiple specialists, or complete medical files that need to be organized when changing cities, providers, or care teams.

For those situations, DyZen Med offers a one-time, per-job option that helps organize records into a clear, review-ready format. The platform focuses on structure and factual organization, while interpretation and medical decisions remain with the appropriate licensed professionals.

How does DyZen Med work?

After you upload records, DyZen Med ingests and organizes the chart—structured summary, timeline, and source-backed review tools help you understand the record before you draft.

The IME workbench then guides you through Review Records, Dictate Exam, Build Draft, Opinion Recommendations, Live Draft Preview, and Narrative Export. Opinion recommendations are AI-generated drafts for physician review; they do not become part of your IME until you accept them. The Narrative Draft helps turn approved findings and opinions into readable report prose—it does not replace your clinical opinions.

Processing time varies based on record size and complexity. Citations and source verification stay available throughout so you can confirm evidence before you rely on any output.
